Fill the online application form at nts.org.pk for the COAVS Lahore / KEMU / Mayo Hospital project within the advertised window, then download and print the filled form. Attach the NTS deposit slip for the prescribed fee, a copy of your CNIC, two recent passport-size photographs and attested copies of all academic certificates and diplomas, and send the complete set by courier directly to NTS at the address printed on the application form so it reaches by 20 October 2026.
